Concierge Physical Therapy Services are provided in the greater Cincinnati, OH area. Please fill out the contact information tab prior to purchasing a physical therapy evaluation or package in order to ensure that we can accommodate you appropriately. Please use key word “Evaluation” in your submission. If you would like to request a specific provider please include their name in your submission as well. Once you submit your contact information we will pair you with a physical therapist who will reach out to you directly with further instructions to set up an evaluation.
We are a cash based physical therapy clinic - meaning your care will not be billed through insurance. While we do not bill through insurance, you are welcome to use your HSA account to pay for our services. Additionally, you are more than encouraged to check with your insurance provider regarding reimbursement for cash based services. Upon your request, within 24 hours of your visit we will provide you with an itemized bill required for insurance reimbursement. This bill can be submitted by you to your insurance at your insurances reimbursement rate.
